The requester’s name.
The requester’s address.
The requester’s driver’s license number, if available.
The requester’s relationship to the voter.
The requester’s signature (written request only).
A request for a vote-by-mail ballot to be mailed must be received no later than 5 p.m. on the 10th day preceding an election.
Obtaining Your Vote-by-Mail Ballot
Vote-by-Mail ballots are mailed approximately 30 days prior to each election to those voters who have requested a vote-by-mail ballot. Vote-by-mail ballots cannot be forwarded. Be sure the elections office has your correct mailing address.
A voter may pick up a ballot for himself or herself at the Supervisor of Elections’ office upon presentation of picture and signature identification.
A qualified absentee voter may designate in writing another person to pick up a vote-by-mail ballot for him or her. Carry out ballots are available to a designee on Election Day or up to nine days prior to the day of the election. The designee may only pick up two (2) vote-by-mail ballots per election other than his or her own, except for ballots picked up for members of his or her immediate family. The designee must provide picture identification and the written authorization from the voter and must complete an affidavit, available on our website at https://www.jacksoncountysoe.org/Portals/Jackson/Documents/Affidavit%20to%20Pick-Up%20Vote-By-Mail%20Ballot%20for%20a%20Voter.pdf
Early Voting
A qualified voter may vote in person at the office of the Supervisor of Elections, Graceville City Hall, or Sneads City Hall; dates and times for early voting are announced by the Supervisor of Elections prior to an election. Before receiving the ballot, the voter must present current and valid picture and signature identification, such as a Florida driver’s license or a Florida identification card.
Voting the Vote-by-Mail Ballot
The voter must personally vote the ballot (unless assistance is required due to blindness, disability, or inability to read or write) and sign the return envelope.
Returning your Vote-by-Mail Ballot
Voted vote-by-mail ballots must be received in the Supervisor of Elections’ office no later than 7 p.m. Election Day. A VOTED VOTE-BY-MAIL BALLOT CANNOT BE ACCEPTED AT THE POLLS.
If you request and receive a vote-by-mail ballot and later decide to vote early or vote at the polls, please take your vote-by-mail ballot with you to be canceled at the early voting site or your polling place. Failure to return your vote-by-mail ballot could cause you delays in voting early or voting at the polls.
How to Correct a Missing Signature on Your Vote-by-Mail Ballot
If you returned your vote-by-mail ballot to your supervisor of elections, but forgot to sign the ballot, your ballot will not count unless you complete and return the Omitted Signature Affidavit for Vote-by-Mail Ballot, available on our website at https://dos.myflorida.com/media/700479/dsde139.pdf (English) or https://dos.myflorida.com/media/701778/dsde139_spa.pdf (Spanish) no later than 5 pm on the second day after the election. Please follow the instructions on the form carefully, as failure to follow these instructions may cause your ballot not to count.
